We are therefore delighted that the first Social Prescribing Link Worker will soon be employed and operational in Southend.

This full-time post, offered by Southend Association of Voluntary Services, is funded by the NHSE through the local PCNs for a year with the possibility to extend.

The role will be to:

Create, develop and maintain solid trusting relationships with GP practices and health professions and voluntary sector partners

Work with and take referrals from wide range of agencies

Provide support to individuals in a holistic manner

The successful candidate will have experience, a sound knowledge of the local area and range of voluntary, statutory and private sector organisations available to support local people.

Full clean driving licence essential.

37 hours per week (to include some evening & weekend working)

Salary: NJC Point 14 – £22,462 per annum
